This project repository provides access to 2 surveys distributed to smartphone-based COVID-19 exposure notification (EN) tool users. Digital EN tools like [WA Notify][1]--launched in Washington State, USA, on 11/30/2020--are privacy-preserving. This makes it challenging to evaluate why users might adopt the tool, how they may respond to receiving an EN, or whether they follow public health guidance to get tested. The baseline survey is made available when users receive an EN, access the WA Department of Health (DOH) landing page (which provides public health guidance after receiving an EN), and opt into a user survey link posted on this page. The follow-up survey is automatically distributed 2 weeks after completing the baseline survey to respondents who agree to receive this survey and provide their email address. The survey files are in a REDCap-uploadable format. Anyone can use these instruments as long as they cite the original contribution. We would like to keep track of modifications or suggested edits and welcome yours. Please send modified versions to Debra Revere (drevere@uw.edu) to post in this repository. We also plan to add a data repository so those working in this area can share survey datasets. The surveys are provided under Creative Commons Attribution (CC BY 4.0).
