Description: Baseline and follow-up surveys utilized to capture experiences with digital exposure notification tools and user engagement in protective behaviors (intended engagement at time of notification and actual engagement 2 weeks later) by those who receive a proximity-based notification that they may have been exposed to someone who tested positive for COVID-19.
