This website offers a list of free resources for researchers and clinicians interested in reducing prognostic uncertainty for their older patients. The CriSTAL tool has been developed and validated for older people aged 65+ years either in residential aged care and hospitals. Some of its components have been used for older patients living in the community and visiting their primary care practitioner. Feel free to modify the databases (XLSX and SPSS or Word) to suit your local resources. However, We recommend you use the tool as provided, without modification, and acknowledge in your work, the team which developed and validated the resources.
