- The following questions should be included in all feedback forms that are used as part of the benchmarking exercise. However, it is up to the institution if they are compulsory to answer or not. - Questions can be presented in any order. - Additional questions may be added to the feedback form. - The five point scale multiple choice answers should be used so that 1 is the worst rating and 5 the best, e.g. 1 - Terrible 2 - Bad 3 - OK 4 - Good 5 - Great Precise wording of the choices is up to each institution. - All feedback forms should contain a **disclaimer** that the anonymised feedback will be shared with other institutions.
