Description: Rehabilitation and other long-time care can be complex processes, where interaction and communication can be core determinants of patient-centeredness and satisfaction outcomes. Today, there is no quality-assured integrated ICT system for preparing, planning, implementing, or evaluating rehabilitation where all relevant stakeholders are included. In this scoping review we therefore want to explore what is known about necessary and sufficient data for patient and providers to define goals and measures, get decision support, and support for evaluation of rehabilitation trajectories. We also want to explore how data should be presented (digital user interface), and in what ways better availability of data contribute to experience with, and benefit of, rehabilitation trajectories.
