**Aim**
We aimed to develop a person oriented, reliable and valid patient reported outcome specific to daily upper limb activities relevant to people with tetraplegia.
The predefined criteria of the new measure were the following:
1. A patient-reported activity measure, explicitly measuring activities using upper limb function for individuals with tetraplegia, typically C5- C8, but also higher incomplete injuries with preserved function in upper limbs;
2. Able to be administered as a questionnaire (paper and electronic), thus enabling remote follow-up outside clinic appointment visits;
3. Fast and easily administered for the individual and clini- cians;
4. Stable psychometric properties, to be used for clinical and research purposes.
