# A clinical pathway for intervention based on cognitive progression Other components of this project make predictions about the rate of cognitive decline in people with long-term conditions using machine learning models. For these models to be translated in to clinic, they need to be embedded in a clinical workflow. In this component project, we will develop a clinical pathway tailored to the rate of change of cognition. ## The benefits of tailored intervention Tailoring information in this way will facilitate the provision of the right support and interventions at the right time. For example, a person with mild stable cognitive impairment may need reassurance and strategies to support every day tasks such as managing finances. These may not be relevant for a person with rapidly progressive cognitive decline who may need interventions such as carers at home or palliative care services. ## Interventions tailored to specific long-term conditions ## In the longer term, we hope to develop specific pathways based on evidence we acquire in specific long-term conditions. For example, identifying people with cardiovascular disease at high risk of cognitive decline in whom intensive control of cholesterol levels and blood pressure could be beneficial.
