Description: Individuals and firms interact with government by way of forms. They serve as portals to applying for benefits, securing permits, paying tickets, etc. A form that is difficult to use can cause annoyance, waste time, and prompt errors, which undermine the quality of customer service; it can also, importantly, prevent a user from completing the form at all, which could deprive them of the benefit or service. For the last two summers, we’ve hosted a first-of-its-kind Form-A-Palooza. Similar to a hackathon, it brought together District agencies, residents, and stakeholders to systematically improve District forms through guided facilitation with behavioral and user design experts. To date, we’ve transformed more than 30 District forms which include the DC Public School Enrollment Packet, Basic Business License, and Driver's License Application.
