COVID-19 Disability Survey -------------------------- The COVID-19 Disability Study involves a 20-minute initial survey and 15-minute follow-up surveys. The surveys include questions about participants’ well-being, lifestyle behaviours, potential COVID-19 related stressors and needs. **Survey development** The survey is developed by the research team in collaboration with partner organizations. The survey includes a combination of questions constructed by the research team and standardized questionnaires. The standardized questionnaires are: - PROMIS Global Health - PROMIS Social Isolation - PROMIS Global Health (parent version) - PROMIS Peer relationship (parent version) - International Physical Activity Questionnaire Short-Form (IPAQ-SF) **Data collection** This page includes the different versions of the surveys (English, French, Plain Language, ASL) for iteration 1 and iteration 2. Data collection for iteration 1 took place between 18 June 2020 and 16 July 2020. Data collection for iteration 2 took place between 16 July 2020 and 18 December 2020. Data collection for iteration 3 took place between 18 December 2020 and 8 September 2021. Data collection for iteration 4 started is ongoing. Survey data have been collected using UBC hosted Qualtrics Tool. For updates on the survey, please visit http://www.disabilitysurvey.ca/.
