Description: Distal pancreatectomy is the standard surgical treatment for resectable tumors in the pancreatic body and tail. While open distal pancreatectomy (ODP) has been the traditional approach, minimally invasive techniques such as laparoscopic (LDP) and robotic distal pancreatectomy (RDP) have gained popularity. This network meta-analysis aims to compare perioperative outcomes among ODP, LDP, and RDP, and identify the optimal surgical approach. A comprehensive literature search will be conducted in multiple databases from inception to December 31, 2024. Randomized controlled trials and comparative observational studies reporting at least one perioperative outcome will be included. Bayesian and frequentist network meta-analyses will be performed to estimate effect sizes, rank treatments, and assess inconsistency. Subgroup and sensitivity analyses will explore potential effect modifiers and robustness of findings. This study, registered on the Open Science Framework, will provide an updated synthesis of the latest evidence on minimally invasive versus open distal pancreatectomy. The results will inform clinical decision-making and guide future research in this rapidly evolving field.
