Description: The SURC model of care provides standardized patient education to patients referred for systemic anti-cancer treatment in the outpatient setting. It provides a single point of contact for patients and carers to access support for toxicities experienced following chemotherapy through telephone or face to face contacts. It was implemented at Western Health in 2013 and has subsequently been adopted as a model for routine management of patients receiving Systemic Anti-Cancer therapy (SACT) within the ambulatory setting. This project aimed to determine whether the SURC model of care is adaptable to other services and to measure acceptability and feasibility: • patient and clinician experience • patient utilisation of a SURC model
